Notes:

For jacks 1 to 3 the first four phone lines are daisy chained together, eg. the blue pair goes to blue on each of the three jacks etc... Likewise for jacks 4 to 6 with lines 5 through 8. Plugging a standard patch cable into one of these jacks, and for example jack 19, will bring the first three lines (blue, orange, and green) to that jack. To rearrange the line order you would need to crimp a custom cable.

On a switch you need to create a vlan on it even if there are no ports assigned to it, if you want it to pass on through a trunked port on that switch.

Interface em0 on the router goes to the first modem.
